Primary Practitioner¡¯s Recognition of Comprehensive Management for Appropriate Medical Services

Abstract
Objective : The purpose of this study is to determine the level of the practitioners¡¯ recognition of Comprehensive Management for Appropriate medical services (CMA).

Materials and Method : A survey was conducted on doctors in private practice and the overall response rate was 22.7%(258/1135). The questionnaire about CMA was composed of 6 parts according to the subjects such as practitioners¡¯
insights for CMA, favorable effects, negative effects, intervening methods of CMA, practitioners¡¯ demand for CMA and its influences on the practitioners.

Results : This survey showed that most of the practitioners took CMA as the government¡¯s restriction for the medical costs. They thought the most favorable effect of CMA was understanding the government¡¯s intension in advance and the worst effect was the downward equalization of the medical practices. Most of the practitioners also thought that the proper intervening method of CMA was a telephone call and the needed improvement in CMA was intensifying the informational provision for them. According to this survey, CMA¡¯s influences on practitioners¡¯ practices were as follows: medical cost per visit day(3.1), prescription rates of injection drug(2.7), prescription rates of antibiotics(2.0), prescription cost per patient(1.5), days of practice per patient(1.1), medicine cost per day of prescription(0.6) as scale from 0 to 5.

Discussion : In general, many doctors in private practice have a feeling of being victimized over almost all parts. This survey shows that the active reflection of practitioners¡¯ opinions on CMA is important in the future of national health care system.

Conclusion : These results can provide us with some guidelines on the reformation of CMA from now on.
